MEMBERSHIP CANCELATION

By completing this form you are requesting that your membership’s monthly draft to stop. Requests received by the 20th of the previous month will stop the next month’s draft per your draft agreement and you will have access to the YMCA until the last day of the month. For example: your request must be in by August 20th to not be draft on September 5th. Your membership is then good until the last day in August.
